Planned Absence Notification Form

Use the form below to request to skip one or more lessons/classes in a future month.

Important Note: Use this form to notify the school of planned absences before the next month’s billing cycle and request that the affected lessons be skipped without charge.

For all other absences, including those due to illness or personal scheduling conflicts, please contact your instructor directly. Make-up music lessons and take-home projects for missed art lessons are encouraged but cannot be guaranteed.
